Papier blanc de forme rectangulaire plié en deux. Au recto, une reproduction photographique d'une scultpture représentant Vierge à l'Enfant sur fond rouge est imprimée du le papier cartonné collé sur le papier cartonné blanc. A l'intérieur sur la page de droite, une inscription est imprimée en bordeaux complétée par une inscription est manuscrite en bleu.
