C'est une plaque de cuivre doré, rectangulaire La plaque porte une inscription en creux, qui occupe la totalité de l'espace. Cette inscription est peinte en noir et est accompagnée d'une flèche, dans la partie basse, indiquant la gauche. Elle est percée aux quatre angles. Au dos, en bas à gauche, de la plaque se trouve une petite étiquette en papier blanc, avec une inscription au feutre noir.
