Pochette en carton vert et blanc, prenant la forme d'un dépliant à trois volets, fermée par un rabat se glissant dans une encoche. Fermée, la pochette présente le nom de Sida Info Service en noir surmontant le dessin d'un teléphone vert et blanc. De nombreuses indications figurent sur les autres volets, ainsi qu'une photographie présentant trois personnes. Sur le volet intérieur, un petit pin's ruban rouge en métal est fixé sur un rabat.
