Papier blanc de forme carrée plié en deux, formant un livret vertical avec impressions. Sur la couverture une photo avec table dressée avec plats multiples dans une salle de réception avec un lustre. Sur chaque côté, motifs imprimés de volutes grises, également imprimés au dos. Textes imprimés sur la double page au centre à l'intérieur, décrivant le menu.
