Plaque de bois rectangulaire, présentant un décor peint : dans un médaillon central figure un soldat barbu fumant, de trois-quarts, en buste, portant un uniforme bleu et une casquette rouge de soldat français de la Première Guerre Mondiale. Quatre écoinçons bruns, sur fond beige, présentent des inscriptions.
