Livret très épais présentant une longue feuille en carton pliée en accordéon et perforée de trous rectangulaires plus ou moins longs. La face présente une grande étiquette en papier beige, ornée sur le coin supérieur gauche d'un carrousel imprimé à l'encre bleue. Des inscriptions y imprimées et manuscrites à l'encre bleue.
