Ce dépliant rectangulaire format portrait est constitué de trois volets. La couverture comporte un fond blanc sur lequel se détache un grand extincteur d'incendie rouge et noir au centre. Une inscription rouge figure en haut et une inscription noir en bas. Au milieu à gauche, se trouvent deux bandeaux noirs contenant du texte en blanc. Le dos du dépliant comporte un texte en noir avec le titre en rouge sur un fond blanc. A l'intérieur, les feuillets 1 et 3 contiennent un texte en noir avec le titre en rouge sur fond blanc et les feuillets 2 et 4 comporte un texte en blanc sur fond rouge.
