Ce dépliant rectangulaire format portrait est constitué de trois volets. La couverture comporte un fond jaune avec un bandeau bleu à gauche. Le titre et le logo sont inscrits en haut et une autre inscription se trouve en bas. Le dos du dépliant contient une inscription en noir sur fond blanc. En bas, se trouve un bandeau bleu . L'intérieur du dépliant est doté d'informations sur l'association en question inscrites en bleu sur un fond jaune. Un bandeau vertical bleu figure le long de la bordure gauche du premier feuillet. Le quatrième feuillet comporte une inscription noire sur un fond blanc. Le bord droit de la page est pourvu d'un bandeau vertical jaune.
