Médaille de forme octogonale, en métal doré, présentant en son centre un motif imprimé : un char d'assaut devant une cathédrale. La médaille est montée sur un ruban tissé rayé blanc et rouge. Le ruban est monté sur une barette dorée, portant une inscription et une épingle à nourrice.
