Petite brochure en papier cartonné, rectangulaire, pliée. La couverture présente de nombreux titres, partiellement contenus dans un cadre rouge. Dans le coin supérieur droit est imprimé un coeur rouge stylisé. Sur le volet gauche, à l'intérieur, sont imprimés des textes, tandis qu'à droite figurent des tracés d'électrocardiogrammes. Un feuillet en plastique, collé sur l'intérieur, se superpose à ces tracés et en présente d'autres.
